LOOK AGAIN – A Learning Lens for Better Investigations

If investigations keep turning up  “human error,” “procedure not followed,” “poor posture,” or “wasn’t paying attention,” it may be time to LOOK AGAIN.

We investigate because we genuinely want to prevent the event from happening again. Yet it’s easy to focus on the most visible part of an incident—the person closest to it. Perhaps a mistake was made, the  wrong action taken or a step was missed.  The investigation is completed, corrective actions aim to correct the mistake…and sometimes a remarkably similar event happens again.

What if better investigations don’t start with a better form or a more sophisticated tool—but with curiosity?

In this fast-paced, interactive session, Heather introduces a practical learning lens for workplace investigations. Rather than starting with “What went wrong?” or “Who didn’t follow the procedure?”, participants will explore better questions and how they can reveal how work happens, why people’s actions made sense at the time, and what workplace conditions influenced the outcome.

Through short examples and lightning-round activities, we’ll explore barriers that can limit what we learn and we'll practice simple ways to LOOK AGAIN: beyond the person, beyond the obvious, and beyond the first explanation.

Participants will leave with a practical new lens they can apply in their very next investigation.
